Your form will be reviewed by a clinician in our Triage Hub and you will be contacted with advice and/or an offer of an appointment when needed. 

The average time for an initial response to requests is 3 hours, but may take up to 24h (1 working day)

Please complete the form with as much information as you can, as this will help the clinician to assess your problem properly.

We recognise that all problems are important, but we need to prioritise those with clinically urgent need.

If the clinican triages your problem as urgent, you will be offered an appointment on the day or within 72 hours. For more routine problems you may need to wait longer, but this will be within a 2 week period.

The online form is open 7am-4pm Mon-Fri. If you need attention after this time and before 6.30pm, call our reception team for advice.

You can also use this form for Advanced Nurse Practitioner (ANP) appointments and Admin requests such as sick notes.
